Peer Evaluation Form Bushman Rev 1/97

Part 1: Your team must get together to agree on the allocation of points on this one line. Share your rationale with each other. Your team has 100 points to allocate for each person. If there are 5 members then you have 500 points (5 x 100) points to allocate. The sum across must = 500 points. One person could receive 180 points while another gets 60.

Part 2: Each member alone rates each member of your group on a 1 to 10 basis for each of the five attributes listed below. 1=little or no accomplishment, 5 = average performance, 10 = outstanding performance. Most scores should be near "5". Give only integer values and do not give the same total ratings to any two members, e.g., you cannot have any two members getting a total of "30" You must break the ties. For each rating below a "4" give a specific example on the back of this form where the person performed below average expectations. Part 2 can be shared in group or in a private discussion with each member. Give each person a copy of your filled out Peer Evaluation Form.

Attribute 1. Promoted positive group atmosphere e.g., promoted a sense of trust, listened, motivated us, resolved disputes, eased tensions, cooperated, facilitated cohesion, was not aggressive, constructively confronted group or individual members to get at the truth and promote excellence, respected the inherent worth and dignity of each member.

Attribute 2. Fostered effective and productive group e.g., understood and adequately did what was needed, urged us to stay with a constructive process without being tied to having things his/her own way, kept us on track, took on assignments, shared relevant ideas, was reliably well prepared.

Attribute 3. Facilitated efficiency e.g., did not waste our time, contributed in a timely fashion, was prompt for meetings, participated fully, was not disruptively oppositional, was not overly competitive, did not try to control group, helped us integrate ideas.

Attribute 4. Contributed a fair share, e.g., took initiative, adequately did work on time, was reliable in the quality and quantity of work, volunteered to do work, shared efforts and didn't withhold ideas or efforts or leave meetings early, carried his/her own weight.

Attribute 5. Demonstrated leadership, e.g., inspired a shared vision, urged us to experiment and take risks to achieve excellence, sought contributions according to skills, was sensitive to the needs of the group, affirmed and acknowledged contributions of members, communicated constructive ideas, supported others when appropriate.
