Instructions

  1. List REQUIRED readings only.
  2. DO NOT LIST personal copies. Use Personal Copy Request Form.
  3. List ONLY Library books and/or books to order.
  4. The Library will purchase one copy of books it does not already own. The Library DOES NOT purchase textbooks, study guides, etc.
  5. This form permits TEN entries. If you have more titles to place on reserve, submit a second form.
  6. For each line INDICATE ONLY ONE LOAN PERIOD. List identical titles with different loan periods separately.
